Insurance & Billing

Ketamine is a proven safe and effective medication for mental health treatment; however, it is not currently covered by most insurance companies. Nonetheless, some of our patients have reported receiving partial reimbursement. As an out-of-network provider, Elevated Health can assist you by providing claim forms to submit to your insurance company. We have observed a changing landscape, with certain insurers beginning to offer partial reimbursement, and we remain optimistic that this trend will continue to expand.

  • Initial appointment - $150

Regular Hours: Mon–Thu: 8:00 AM – 4:00 PM | Fri: 9:00 AM – 1:00 PM

Concierge Hours: Available by appointment outside these hours for eligible patients.

  • If you are experiencing a mental health crisis or emergency, call 911 or go to the nearest emergency department. Our clinic does not provide emergency services outside of scheduled appointments.
